直径7.6cm；高2.8cm
|香盒造型规整，木胎轻薄，子母口扣盒紧实。盖作盝顶式，直壁下斜收，底承圈足。通体髹黑漆，漆色锃亮，光彩熠熠，极为悦目。盒面以金彩描绘纹饰，顶面饰五岳真形图，盝顶斜边饰楷书两周，内圈字体较大，文曰“盖闻，乾坤之内，五岳者谓之神。五岳之中，岱岳为其祖，莫不应乎造化。生于混沌之初，自阴阳镇乎。”外圈字体较小，文字分别简述五岳泰山、华山、衡山、恒山、嵩山之概要。|五岳真形之图起源颇早，据记载，“西王母与上元夫人降汉武帝，帝视其巾器中有小书，乞瞻览。母曰：‘此五岳真形图也，乃三天太上所出，岂汝所宜佩乎?’帝乞不已，与之，藏柏梁台”。此外在《汉武帝内传》里面也有记载，称“五岳真形图”是太上老君道德天尊最早测绘的山岳地图。||旧配供盒及包装。
