叶衣母的身黄色，有三面及六臂，其中间面黄色，右为白色，左为红色，每面均现忿怒表情，各有三只眼睛，各口微张而露出四颗尖尖的犬牙。叶衣母的右边第一手置胸前持金刚杵，左边第一只手持勾索亦置胸前；右边第二手持降魔斧头，左边第二手持树叶；右边第三手持一箭，左边第三手持由有花之树木所造的弓。本尊的姿势是半蹲立、半斜坐的，左足蹲伸而右足屈入。这个姿势称为“皇帝坐姿”。在叶衣母身上，佩戴了种种宝石璎珞。他的下身，披围着一件由树叶串编的草叶裙，就像某些国家的原居民之民族服饰般。
