此牌为和阗白玉籽料雕就，玉质白润纯净，细腻如脂。器呈片状，上小下大。牌中葫芦型开光，牌额及外缘以密集的细阴线琢刻如意云纹。葫芦形开光圆润饱满，起细棱边，中部纹饰皆为减地浮雕。正面雕饰一枝挺拔的梅花，枝干苍劲舒展，花朵错落有致，花蕊、花苞雕琢皆一丝不苟。梅树之下生有灵芝及兰草，构图清新淡雅，十分具有文人风骨。背面上下以小篆雕饰“大吉”二字，字体秀气，线条舒展匀称。葫芦谐音“福禄”，象征着和谐美满、兴旺吉祥。整件玉牌形制规整，选料上乘，雕工极为精细考究，底面打磨平整，抛光细腻匀净，具有清宫造办处风格特征，实为一件清代乾隆时期的玉牌精品。
