提要：整幅仿造碑刻形制，“功牌”两个大字居中置顶，周边印有“二龙戏珠”纹饰。题头表明颁发者的衔位品级，正文说明获颁功牌缘由，然后写明奖赏的人员，在“功牌”左侧标注的年月日，上盖有篆书方印一枚。“功牌”字样上有朱笔在上面圈、点、勾画的痕迹。此为时任头品顶戴、湖广总督的段祺瑞颁发给立功军人申泗河的功牌。落款“本总统段”仿佛预言一般，令人感慨。整体品相完好，市场稀有。
