是志刊于隋仁寿三年（603），清光绪十三年（1887）在陕西蒲城县出土。康有为在《广艺舟双楫》中言此志“初入人间，辄得盛名。以其端整妍美，足为干禄之资”。此志楷法成熟，结字谨严，用笔劲利，神采飞动，兼有南帖之绵丽与北碑之峻整，集秀丽与雄劲于一身，实为隋人书法的代表作。又因其字迹清晰完好，成为后世学习楷书的绝佳范本。知县张荣升于志石出土次年（1888）于第三十一行“曰”字下刻跋二行，不久即为人铲去，此后拓本刻跋处有明显铲痕，即为铲跋本。
