From Captain William Swain (1781 - 1846), the document reads "Third Reg. N.Y. This is followed by a table with three columns for Names, Residence and Remarks. The first to sign is William Swain followed by his first Lieutenant and dated January 16th 1812. The volunteer roll of enlistment dates continue until 1814, and include 49 signatures in total. From the Francis Hopkinson Family Archives
