狼五山，又名狼山，位于江苏省南通市，是大势至菩萨的道场，为中国佛教八小名山之一。全志分山图、形胜、山川、古迹、建置、物产、仙释、灵应等目。卷一古迹记述“按州南阇发轫踰六七里，一烟墩再里许有观音堂，傍有勇士曹鼎朱寅墓”为史志关于明代抗倭英雄曹顶墓的最早记载。此本据王培孙藏万历四十四年刊本影印，经查馆藏记录，万历原刻本仅上海图书馆珍藏，传世极罕。据卷前序，此本仅影印五百部，亦不易得。拍品包角尚存，品相完好。
